How to Use the Online Request (ORS)?​

​

  1. Check the calendar to select a desired date for sending the request. (Note that each date has a maximum slot of 50, so ensure that the desired date is available.)

  2. Fill out all the necessary information required for the request.

  3. Click "Submit"

After Clicking Submit: 

  1. Wait for an email confirmation stating that the request has been successfully sent.

  2. If the request is for the release of Transcript of Records (TOR)/Permanent Record/Form 137, the following steps apply: a. After three working days from the date of the request, the TOR will be released. b. Prepare the required amount for the documentary stamp tax. c. Visit the designated location for the TOR release and bring the necessary documents and payment. d. Pay the documentary stamp tax and submit any required documents. e. Receive the Transcript of Records/Permanent Record/Form 137.

  3. For other documents (excluding TOR), they will be available on the date of the request.

  4. Ensure to collect the documents on the designated date and location as specified during the request process.

IMPORTANT: PREPARE A DOCUMENTARY STAMP BEFORE CLAIMING YOUR TOR/F137.  Pursuant to National Internal Revenue Code as amended and reiterated by the Revenue Regulations No. 4-2018 Sec 88 Stamp Tax on Certificates, School Credentials shall now be released with a documentary stamp tax.
