Application for Transfer-In and Balik-Aral Students
Step 0. Fill-Up the Enrollment Form
Download the following forms:
1. Enhanced Basic Education Form
2. Enrollment Form for Transfer-IN
3. Enrollment Form for Balik-Aral
4. Subject Evaluation Form
5. Enrollment Slip
or you can personally visit the Office of the Registrar to get copies. Please make sure that you have a copy of your Form 138 and PSA birth certificate before answering the form.
Step 1. Evaluation of Report Card
Your report card will be evaluated by the subject evaluator.
Step 2. Submission of Enrollment Credentials
The Office of the Registrar will evaluate the following forms and documents:

1. Birth Certificate

2. Report Card (SF9/Form 138)

3. Certificate of Good Moral Character
-
4. Enrolment Form Modified Basic Education Enrolment Form.

If you do not prefer online enrollment, the office of the registrar allows walk-in transactions provided that LGU allows such face to face transactions and with minimum health protocols.
The Office of the Registrar is open from 8:00 AM to 4 PM (Monday to Friday, except holidays)
You can download the Enrolment Form here in .pdf file. You can present this form to the Office of the Registrar.
Step 3. Interview by the School's Principal
You will be interviewed by the school's principal. This is where you enrollment will get approved. (We will inform you about your schedule)
Step 4. Orientation by the Office of the Guidance & Discipline
You will undergo orientation for the school rules and regulations. (We will inform you about your schedule)
Step 5. Creation of your Malvar SHS Account
Your G Suite Account will be processed by the school's web administrators.
Step 6. Endorsement to the Class Adviser
Endorsement to the Class Adviser for Class Schedule and other matters.
